How do I authorize and connect my electronic health record (EHR) to Evidation?
To connect your EHR, you’ll first need to sign an authorization form. This form gives Evidation permission to securely request and use your health information, as described in our Privacy Notice and Consumer Health Data Privacy Notice. You are always in control — signing is voluntary, and you may withdraw your authorization at any time.
Once you’ve signed the authorization:
- Go to the Evidation app and find your Connect your electronic health record (EHR) card.
- Search for your health system(s) or hospital(s) where you receive your care. You can connect more than one provider.
- Log in with your patient portal username and password. Common patient portals include MyChart for Epic and CernerHealth.
- If prompted, select “Allow All” so Evidation can securely access your full record. If you prefer to limit the information shared, you may have the option to select specific EHR data to disclose.
That’s it — your EHR will be linked to your Evidation account. You’ll earn points for connecting, and your data will help support health research.
If you run into issues while connecting, contact us directly from within the app by navigating to Account, then Settings, then Contact Us.
I want to add more providers but I don’t see my EHR card
If you no longer see your EHR card in your Evidation feed, but want to connect an additional provider, please follow the steps below:
- From your home screen, navigate to “Connected Apps”
- Scroll down to “Electronic Medical Records”
- You will see the list of providers you have already connected
- Tap “Add Connection,” and follow the instructions to add new providers to your list
If you run into issues while connecting, contact us directly from within the app by navigating to Account, then Settings, then Contact Us.
How do I earn points for my EHR?
You’ll earn points when you successfully connect your first health care provider through 1UpHealth. Points are awarded one time, even if you connect more than one provider.
How do I disconnect my EHR from Evidation?
To disconnect your EHR, you’ll need to withdraw the EHR Authorization you previously signed. Here’s how:
- Go to your Evidation profile
- Select My Consents
- Choose Evidation EHR Authorization
- Select Withdraw consent
At this time, it’s not possible to disconnect individual health care providers separately. If you delete your Evidation account while your authorization is active, we’ll treat that as a request to withdraw your authorization.
