Thank you for your interest in The GLP-1 Program on Evidation. This program is intended to help you share your experience taking GLP-1 medications. As part of the program, you will be given the opportunity to share your GLP-1 journey, including regular check-ins about things like side effects, changes in your medication, and your overall health. This information can help us identify upcoming research opportunities you may qualify for. Please be aware that this program does not provide real-time monitoring or diagnosis, nor is it intended to take the place of your healthcare provider. Please work with your healthcare provider to understand any changes to your health you may experience and get medical advice.
What do I have to do to participate?
Complete a one-time welcome survey
Complete weekly surveys about your about your weight, experience with food, and side effects
Complete monthly surveys about your overall GLP-1 experience, including any medication changes
Continue wearing and using the wearable devices and/or apps you have linked to your Evidation account (if applicable)
What data may be collected from me or analyzed as part of this project?
Your survey responses, which include: demographic information, as well as basic health information to understand your GLP-1 experience; and
Information from your Evidation profile, including zip code, age, race/ethnicity, sex/gender, and relevant medical diagnoses you have provided in your responses to survey questions (previously and added during the program); and
Responses to Evidation check-in questions, including weekly questions about your weight, experience with food, and side effects, and monthly questions about your overall GLP-1 experience, including any medication changes; and
Data related to how you interact with the program and program activities; and
Your connected activity data (e.g., steps, sleep, heart rate), which will be regularly analyzed during the course of the program.
Who will have access to my data and how will my data be used?
If you choose to participate, we will perform ongoing analyses of the above described data. Limited Evidation team members will have access to the coded dataset (meaning that your directly identifying information is removed and replaced with a number) to conduct analyses. Your data may also be used to operate and improve our services, including development of new products, features and functionalities, and for the design of future programs.
Additionally, if you choose to participate, we may also share a summary analysis of the aggregate (combined) surveys and data back with the Evidation community. No individual information will be shared. We may also share coded data collected from the app and surveys with certain research partners.
As part of your participation, you may be asked to share your coded data with a third party, such as a research partner. Sharing your coded data with a third party is completely optional. We will provide additional information about any data sharing opportunity so you can make an informed decision. Deciding to not participate in these additional data sharing opportunities will not impact your ability to participate in The GLP-1 Program. We will not request additional consent from you to share any aggregated information.
Your responses may also be used to help match you with future surveys or related opportunities where you can choose to participate and earn rewards.
What if I want to stop taking part in The GLP-1 Program?
Participating in The GLP-1 Program is completely voluntary and you are always free to leave. You can unenroll at any time by navigating to your Evidation Profile and accessing your current programs. If you choose to stop taking part in The GLP-1 program, your data will no longer be collected through the program.
Changes:
Evidation reserves the right to modify or terminate these terms, with notice, at Evidation’s sole discretion. The modified version of the terms shall be available to you [LINK HERE]. Please note by continuing your participation in the program you are agreeing to the program terms.
